 4.0.2 - SQL errors after installing TemplaVoila 0.4.0 - HELP

After installing TemplaVoila (which looks like my only hope in getting typo3 to work, as the base template system is far too complicated), when I click on file > filelist module > template/mail and click the icon of my template, I get a java script error instead of a drop down menu.
I also get SQL errors on step 3 if I try to run the TemplaVoila wizard.
Please help!
